As with any successful project, the journey for the CaReMe Model of Care transformation starts with initial desk-based ‘homework’. Reflect on your main priorities and how to address the specific needs of your environment – followed by the engagement of your Core Team.
We are aware that there is no ‘one-size-fits-all’ solution. Each institution has its own needs, ‘pain points’, priorities, strengths, weaknesses, etc., so the model of care described in this guide could be considered somewhat ‘universal’. However, to ensure success: you must take the lead, engage your team and get to a point where you – and your organisation – own the design and implementation of your tailored CaReMe solution.
Learning from our struggles and leveraging our experiences and expertise within this specific field, we help you navigate the complexities of delivering the CaReMe Model of Care. The ‘Step-by-Step’ approach is simple and easy to use with unique tools and templates.
You are guided through the 8 macro areas, which define the WHAT, the WHERE, the WHO, the WHY, the WHEN and the HOW.
For your convenience – templates are available to download – for printing or editing.
Those who prefer the ‘old-school’ method of Pen & Paper (P&P) – downloadable templates are provided to facilitate your brainstorming sessions. For the more tech-savvy, there is a complete editable project plan slide deck with the same step-by-step approach of the templates.
Ultimately, choose what suits you best; P&P vs. Digital Approach. Or a mix of both.
Use third-party software to structure notes more dynamically and visually (e.g., using mind maps) or to simplify teamwork and documents’ sharing (e.g., Teams, OneDrive, Dropbox, etc.). We’ll also provide links to some of the many options, often freeware, we find useful and easy to use.
In preparation, it is advisable to do some additional reading – to have an effective mindset and to learn general techniques. “Tiny Habits” by BJ Fogg (2019) is a great place to start when aiming for behavioural change.
