By completing this task, you will have:
Aim: To determine your short-, medium- and long-term plans.
Objective: Prioritise areas for short-, medium- and long-term improvement according to: relevance, feasibility and probability of success.
Method: Refer to the previous exercise and templates – used for the prioritisation of Goals and Actions and the GANTT chart. Check that everything is captured consistently and distributed over time – accordingly with clear RACI assignments.
By completing this task, you will have:
Recap: At this stage of the process: you have answered all the “W” questions.
Aim & Objectives: Develop a Short-term Plan Summary & “Elevator pitch” with the aim of obtaining the necessary approvals. Includes: hospital management & dedicate staff resources of your CaReMe clinic, the budget of necessary facilities & personnel, tools and/or equipment required, etc.
Method: Put all the insights from previous steps together to develop a short-term plan for how to get started with your CaReMe clinic. (If not, it may be an opportunity to revisit some of the previous steps first).
By completing this task, you will have:
Aim: Create SOPs & documents that will govern how your CaReMe clinic operates. This is crucial!
Objective: The procedures should determine the performance of your clinic – in practical terms: to ensure consistency in patient management & how other departments collaborate to attain the best results in a cost-effective manner.
Method: Documents should describe how to handle the ‘patient journey’: from the moment they encounter your CaReMe clinic, until they are no longer under your care.
