STRATEGIC S.M.A.R.T. PLANNING Steps

By completing this task, you will have:

• A set of structured short-, medium- and long-term plans
• Filled in the specific slides on the project deck

Aim: To determine your short-, medium- and long-term plans.

Objective: Prioritise areas for short-, medium- and long-term improvement according to: relevance, feasibility and probability of success.

Method: Refer to the previous exercise and templates – used for the prioritisation of Goals and Actions and the GANTT chart. Check that everything is captured consistently and distributed over time – accordingly with clear RACI assignments.

By completing this task, you will have:

• A clear short-term plan that summarises the key aspects – from each of the eight Domains: i.e., detailing your key goals, proposed target patient population, operational structure, plans for integration of technology, team, and key timelines for the implementation
• A short-version of your plan – the so-called “Elevator pitch”, which can be presented in a maximum of 3 minutes
• Made sure it was developed in a format that is easy to share (use the templates in .ppt or .doc)

Recap: At this stage of the process: you have answered all the “W” questions.

Aim & Objectives: Develop a Short-term Plan Summary & “Elevator pitch” with the aim of obtaining the necessary approvals. Includes: hospital management & dedicate staff resources of your CaReMe clinic, the budget of necessary facilities & personnel, tools and/or equipment required, etc.

Method: Put all the insights from previous steps together to develop a short-term plan for how to get started with your CaReMe clinic. (If not, it may be an opportunity to revisit some of the previous steps first).
